    建于2013年7月，业务用房面积1085平方米，下设宅库、徐家疃2处社区卫生服务站和牛角沟、闫家庄、西山口、西庄4处村卫生室，承担嵩山街道办事处19个村（居）1万余居民的预防保健、基本医疗、基本公共卫生服务。年内，在职职工19人，其中中级职称3人。设有全科门诊、中医科、中西药房、针灸理疗室、养生保健室等10余个科室。年门诊量3万余人次，业务收入15万元。
