    金泰和元年（１２０１年）立于岱阴谷山玉泉寺西侧山岗，
党怀英撰书并篆额。碑高２米，宽１米，厚０。２米。碑文隶书１８行，满行
５１字。碑阴篆额“玉泉禅寺勤迹檀那记”；题名４４行，满行９０字，正书
，均为捐施者姓名。碑文主要叙述寺院沿革及金代住持僧善宁、法郎、智崇等
人相继重修、拓建寺院和“勤苦作劳而无怠意”的经过。今碑完好无损。
