    位于岱阴柳埠村东北角的山崖上。坐北朝南，南北长１３．８米
，东西宽１２．５米，总面积为１７２．５平方米，庙前台阶１０８级。该庙
建于明隆庆四年（１５７０年），为硬山式砖木石结构，小灰瓦覆顶。前有山
门１座，面阔３间，进深１间，卷棚顶。山门内为东西配殿，前出厦卷棚顶，
面阔各５间，进深２间。后为大殿，面阔３间，进深２间。殿内墙有壁画，画
面已不清。正脊檩题“皇清乾隆四十六年岁次辛丑桐月下浣谷旦重修”。西配
殿墙上嵌石碑１块，上书“隆庆四年十月初六日建立，领袖张贵、董民”。
