    位于万德镇史庄村西北首，为单檐硬山顶建筑。面阔３间，进深
１间，全灰布瓦盖顶，内有宽９．４米、长５米、高４．２米的清代壁画，画
面清晰。另有明代创建三皇庙题记１方。该庙为明万历三十五年（１６０７年
）建，现仍保持原貌。
